1.                 Nominee must have been a resident of Lee or Collier County for at least five years.

2.                 Nominee must be a member of the Bonita Springs Area Chamber of Commerce.

3.                 Nominee must have actively participated in the Chamber as an officer, director, or committee member.

4.                 A detailed outline of the recipients community service work, professional accomplishments and contributions to the community must be submitted with this application
